It may have been when the switch from steel cuffs to plastic was being made:

Cant see them removing steel cuffs THEN placing plastic restraints on in that order.

I cant see how proper procedure could have allowed this to happen.

I recently watched a show on the History channel about the Aryan Brotherhood. They showed how they train their members how to escape restraints and often carry a handcuff key. They even have a way of snorting the key up their nose! Among other places

The guards basically said that the restraints are nothing more than something to give you that extra few seconds to respond to a threat. It is no guarantee.
