Class Completion Form

What is the Certificate Number? On the form underneath that column it says (fail). I cant find this in my forms booklet and dont remember it being covered in class although I am sure it was.

numbers was used on the older forms , so now just put pass in that slot or if the student fails write in fail :tiphat:

To be honest with you - I've never submitted a class completion form.

I have a question regarding this form so I will post it here.
For the past 2 classes, I've signed, scanned and emailed the form back to them. Today, I got an email back saying that we always have to mail this in. Not to argue with them, I agree. Is this true?

That's what they said in our class, Kinny. Seems they want your "original signature" or some foolishness like that. :fire

(Besides, they want to let the Postal Service have a chance at loosing it. :smilelol5: )

You need to start! One of the changes coming for instructors is being able to do this online and this will also mean we don't have to students a CHL-100.

This is correct. I also fax or email a courtesy copy to show I did send it in, just in case the mailed original doesn't make it.

I'll have to go back and double check, but I thought the book from Instructor school specifically mentioned faxing as an option.

In my class book, beside the slide for the "Certificate of Class Completion", my notes state: "Must be SNAIL mailed"

Edit: I see the second slide on the page says: "ONLY accepted by Mail"
