Class Completion Form

What is the Certificate Number? On the form underneath that column it says (fail). I cant find this in my forms booklet and dont remember it being covered in class although I am sure it was.

numbers was used on the older forms , so now just put pass in that slot or if the student fails write in fail

I have a question regarding this form so I will post it here.
For the past 2 classes, I've signed, scanned and emailed the form back to them. Today, I got an email back saying that we always have to mail this in. Not to argue with them, I agree. Is this true?

That's what they said in our class, Kinny. Seems they want your "original signature" or some foolishness like that.

(Besides, they want to let the Postal Service have a chance at loosing it. )

To be honest with you - I've never submitted a class completion form.

You need to start! One of the changes coming for instructors is being able to do this online and this will also mean we don't have to students a CHL-100.

This is correct. I also fax or email a courtesy copy to show I did send it in, just in case the mailed original doesn't make it.

I'll have to go back and double check, but I thought the book from Instructor school specifically mentioned faxing as an option.

In my class book, beside the slide for the "Certificate of Class Completion", my notes state: "Must be SNAIL mailed"

Edit: I see the second slide on the page says: "ONLY accepted by Mail"
