Actor Jon-Erik Hexum was killed on a movie set by a verified blank round. The powder charge propelled the wadding thru his skull, pushing a chunk of bone, around quarter-sized, into his brain.

Yes, I remember that. I could not remember the guys name. Thought about it when reading this post. As I recall the gun shot was self inflicted.
