Name Change Documents
Does anyone know where I can get the photo signature card other than it being mailed to me from the state? I recently got married and have changed my name on my DL. I was told by the CHL office that CR-70 was the form to print online and send in. Well I printed the form, but it clearly states that I must send it in with a new photo card and signature. When I called back they didn't realize that, so said they would mail me out an entirely new application and I could use that card. Well that was over 3 weeks ago now so when I called back, they said they would mail the signature card again. Since the law requires me to make this change so quickly, I am rather concerned at this point. Help would be greatly appreciated.

Do you already have your CHL? If so, I think that you can change your name online.

Find a CHL instructor. We all have photo card holders.

I do already have my CHL. I was told that only address changes can be done online, since I have to show proof of the name change.
I am in Austin, but thank you very much for the offer Crossfire. I will try and get a hold of my instructor from my renewal class last year.
