I had this exact same problem. I then ordered a new matching hammer and disconnector and dropped that in. Problem Solved. I bought RRA parts. When that happened to me, I believe it was the disconnector out of spec (machined wrong). I want to believe that the hammer and disconnector when replaced needs to be replaced together as a set. RRA disconnect and Bushmaster hammer might not work per say...
