Company name change?

I started with my company in 2000 and they have had 3 different names in the last 5 years (the company was bought out). Do I list my company's current name and 13 yrs employment, or break it all out and it would look like I've been been with the "current" company for only a few months?

Same address? I would list the current name, with prior names in parens.

No each time the company was bought it was buy a company in another state, so company address has changed a few times.

since it's different addresses, I believe you treat it as different companies, that happened to my husband and I'm pretty sure that's what he did, but it's been a while and he's not here right now.

Are you working in different locations or is the company headquarters in different locations? I'm pretty sure DPS wants the address where you work, not corp hq in NY.
