Fast form

Hello, I am about to send in my packet and have a question about the FAST form. Do I need to include this in my packet? Also I noticed on the form my name is all rearranged middle is my last, last is my first, and first is my middle. I told the person at L-1 and she changed it on screen. Am I good to go?

The L-1 system frequently rearranges your name for you. No one seems to know why. If it got fixed in the system when you were printed, then you are fine.
The FAST form does not go to DPS. You keep that in case they say they did not get your prints. It has your TCN number, which you would need in that case.

I always believe more is better, just like the background information. I would send a copy.
I can attest that having to provide additional information after submission sucks...
