You need to go back to the county(s) where you were convicted and get a disposition record of that arrest and conviction. If you're a patient man, you can send exactly what the instructions say to send, and then wait for DPS to tell you exactly what else they want.

Will the states send me my records cause I sure can't afford to go get them being that they are in Vermont and Arizona.If I list what I know to the best of my knowledge how will that work for me?
