Renewal CHL Checklist

I'm about to send this in, but I'm unclear on the checklist needed for the renewal. I only see a checklist for an initial application.

I have the following:

Fingerprints on the white card with blue outlines
Two passport photos in the little holders
The "Photo Holder", with my signature at the bottom, inside the rectangle.
Notarized authorization for release of records
Notarized "Knowledge of Laws Affidavit"
Notarized Eligibility Affidavit
Certificate of Training form

The instructor said that renewals don't require the yellow application sheets, and that we're only required to submit one set of fingerprints.

I have everything else on the blue "document checklist". It just seems strange to me that there isn't a checklist specifically for renewals.

Edited to add: I just completed the application online, paid the fee via credit card, and printed my receipt.
