Have you already applied with DPS online? If you have, then you use the "Contact" option on one DPS page. If you have not applied with DPS yet, then you cannot upload it until after you have done so. I've provided some links below.

Thanks for the info, I just uploaded it

I forgot to warn you to be sure to press the "Attach" button after selecting the document to upload. That button is at the bottom of the string of "Browse" buttons and easy to miss.
