I'm joining the line-up guys...
I got these forms from the Medical Advisory Board. They were printed out with my name and application number on it. If someone needs them I suggest contacting their State Representative like I did. Without these completed forms, and possibly even with them, you are probably adding an addition two months to your wait. Don't ask me how I know. I got my forms after getting my State Representative involved with finding out the status of my application.

How long does it generally take to get a response back after that? I did get one after I sent in the info requested, but nothing as far as the results yet. IS DPS likely to call me?Dear Mr. ____,
I would be happy to check on the status of your application- I just need your Texas driver's license number and your birthdate. Thank you for your understanding of the backlog that DPS is working through-their applications have shot up over 100% since the start of the new year. When I have your information I will get it submitted promptly.
