Looks like he had a revolver, but i cant tell if he was a bad shot or he had a problem with the gun.

You are correct, it is a revolver and he is also a bad shot.
When he shot the police officer, he had a white rag wrapped around the gun either to disguise it or to keep his fingerprints off it. Then it looked like he was going to wipe the gun off with the rag (and possibly throw it down and run). He gave up whatever idea he had when others started going after him and began shooting others.

It appears that he ran out of bullets and kept pulling the trigger with no effect for awhile. Then while he was inside the subway car he obviously reloaded as he shot at least once out the door.

Also, did you notice that he came back out of the subway car and stole the laptop or messenger bag that was laying on the ground that belonged to the guy in the white shirt he shot?

The guy who came out of the car and tried to stop him, although usuccessful, definitely had a fighting spirit. It's very interesting how some people walk around during the incident and are totally oblivious to what is happening while others are running for cover.

All guns have at least two safeties. One's digital, one's cognitive. In other words - keep the digit off the trigger until ready to fire, and THINK. Some guns also have mechanical safeties on top of those. But if the first two don't work, the mechanical ones aren't guaranteed. - me

I'm pretty sure that's just his reflection on the side of the subway.

If the man who tackled him was just armed... :grumble
