This is the way I was taught when you have a shirt over the pistol/ holster, so YMMV.
With your support hand, grab the strong side of the shirt and lift up while your strong hand starting to draw.
Keep your support hand as close to your body as possible. After your pistol clears the holster proceed with normal drawing position.
That's the best way I could explain it.
Maybe I should make another animated gif on this.

Smurf,
This is a great subject. I hope all here will take time to post their concerns and remedies.
Naturally, where/how you carry, the type weapon, hoslter, and clothing you wear will present different challenges.
Two very common things I see are: "Strap Traps" where a holster fitted with a retention strap is utilized, and "Garment Stalls/Traps".
Strap:
When trying to draw quickly or simply under pressure, it is easy to "pop" the snap, grab the pistol...BUT leave your thumb on the strap during the initial draw. This usually results in only a "stalled" draw, but someone in a full blown panic might not figure out what they are doing in time.
Garment:
Another common problem occurs when any article of clothing (shirt, blouse, vest, jacket, etc..) is allowed to hang loose near the grips, or is not properly "cleared" prior your draw. What happens most often is: You end up with a handful of garment and grip!

The outcome of course, is a slower draw (at best). In fact this happened to an old knife instructor of mine recently when when we got together to shoot with Longtooth.
He (instructor) had allowed his shirt (tucked in) to become blousy on his strong side, when he reached down to draw...he pushed part of his shirt down on top of the grips. It wasn't too pretty.
Anyway, I'll shut up now and see what others have to contribute. It is well to consider what works best for you, and you have shown yourself astute by recognizing the need.

The garment/grip thing was a problem for me in the beginning. I usually wear a sleevless athletic type shirt underneath a carhart short-sleeve shirt for summertime.
I had to find an undershirt that was cool, and didn't blouse when tucked. With the overshirt, I find that using a couple stitches to sew a lead fishing weight into the hem helps it to get out of the way. With the weight, when you flip it back, it carries more of the shirt with it. Thus allowing you to grip the gun only.
It also requires more of a breeze to flip the shirt open and unconceal your weapon. I actually put the weight in for this purpose, and found the other benificial as well.
Does that make any sense?
As for drawing while wearing a T-shirt only to conceal....well, I can't offer much there. My H&K is way too big for that, and I carry my P3AT in my pocket when using it.

When wearing a baggy t-shirt, here's my technique:
1. Drop strong hand down to thigh.
2. Extend thumb straight out, against thigh.
3. Sweep back and up, keeping thumb in constant contact against thigh/buttock, "hooking" the shirt up.
4. Continue the sweep up behind the holster, then forward over it, forcing material down between holster and body, as I establish a grip.
5. Draw.
It's more complicated to describe than to do. It works well, but I'll never beat Bob Munden to the draw.
I don't involve my weak hand in the draw. I keep it bladed, right in front of my chest, to use for deflecting a blow, thrusting out to warn someone to "STOP!", etc. As my gun hand comes up, I move my weak hand into the support position.

Actually, I could tell. And I don't know which is more sad - that I knew what it was or that flintknapper didn't.
