yes actually they are ALL preventable, open bolt use the correct ammo not some cheap junk, disconnector is worn replace it, maintain your equipment not your excuses again there is NO SUCH THING as an accidental discharge of a firearm period

I cannot honestly say that I take micrometers to each part in my firearms when I clean and maintain them. With the tolerances we are often dealing with, I am not sure how else I could fit your criteria. I hope that it is never an issue for me.
