I would also start looking for a new job. I would seriously consider not giving advance notice when I changed jobs. Texas law does not require it and a company like that hasn't earned it.

I could be wrong, but I'm pretty sure it's illegal for them to ask any of this as a condition of employment....this stuff is insurance stuff, medical stuff, it has absolutely nothing to do with how good you are at your job.
