The only one that comes to mind was recent at a training class. We were practicing one hand shooting, weak side. I have a 1911 but the safety is not ambidextrous. So, I tried flipping it down with my trigger finger (finger off the trigger, right?) but my middle finger followed the trigger finger up and slipped into the trigger guard when I released the safety, setting off a round. The gun was pointed down range but I think it went over the berm.
I am going to put an ambi safety on that gun first chance I get. The grip safety was normal as well, not taped down as I see a lot of people do.
