My personal opinion from a lot of years watching, reading, sometimes being involved in the after.. is, two elements are often true…
1: The gun went off
2: The gun was dropped
However, most often the missing third element not reported is, the trigger was pulled.

Making the true sequence a little different.
1: the trigger was pulled
2: the gun went off
3: the gun was dropped.
