I have the same problem with my PT145; only while firing. I think it has something to do with how the mag release sticks out. If I don't watch my grip I accidentally push the release. One of the mags that came with the gun was faulty and Taurus replaced at no charge with a 10 day turn-a-round. I had minor problems with the magazine randomly being ejected both while shooting and while the gun was in a holster. The problem was serious enough that it caused me to not trust a PT145 as my carry weapon. The pistol was looked at by a local gunsmith who could not find any issues, but I never sent it into Taurus. Instead, I traded the gun in for another one.
On the plus side, the pistol was very accurate and I really liked shooting it aside from the magazine ejection problem.
My intention is to eventually replace it with something like a Kimber Ultra Carry II with Crimson Trace laser. My Para GI Expert has converted me into a 1911 fan.
