You could have an issue with the hammer release lever. Maybe it got bent and is flipping the safety on during recoil. Odd, but that's the only thing I can think of unless your safely/decocker lever is broken. Double check that your hammer release lever on the frame is not bent or broken. If you lock the slide back, is the safety/decocker easily manipulated? Might check your safety plunger and spring to see that they're installed correctly as well.

So, is it the safety/decocker actually moving to the "ON" position or is it that the hammer decocks after a shot? It's possible to have the SA notch of the hammer wear or have issues with the sear/sear spring which allow the hammer to drop after a shot which would give the shooter the idea that the safety/decocker lever had been actuated. The sear is not used in a DA shot on the 92, only in single action, so if the hammer is following the slide forward, I'd look at the hammer/sear contact areas for your problem. Otherwise, if the safety/decocker IS flipping somehow, lock your slide back and see how freely it moves. Could be another issue.
