For my family doctor, I don't mind them knowing and would carry on body. If you want to remain concealed, that's a great time for an ankle or pocket BUG. I've also used the backpack carry option. It used to be my default for most of my initial license term.

I've heard whispers that our hospital is posted, but I always enter through the "professional office building" (~6 story doctor offices attached to the hospital on two floors), which is not posted, and I've never been notified that I'm not welcome there. I definitely feel no compulsion to circumnavigate every building I enter inspecting every doorway on a treasure hunt for signs. YMMV.
