I've found that a light, white nylon/velcro shoulder holster is very effective in completely hiding a handgun in the space between my armpit and breast pocket. If I'm wearing a tie, I just leave two buttons un-buttoned. That let's you reach into your shirt to grip it without exposing anything. Works fine whether sitting, driving, or standing. My left arm and/or glasses in breast pocket generally cover the handgun to prevent people from bumping or hugging it. So for me it is the best compromise of deep concealment and accessibility.
