I received it as well. I just did a class of 17 last Sunday. When I saw the first date my heart skipped a beat.....

"As of July 24, 2017, the form, CHL-100, will now be LTC-100. Please begin using this new form immediately. After September 1, 2017, any CHL-100’s received by the department, dated after September 1, 2017 will not be accepted and that application will be rejected. This will then require your students to return to you for the new form. If you have any questions, please feel free to email them to RSD_LRS_CHL@dps.texas.gov.
