If it was indeed a NAA mini revolver, I'd wager it was carried with the firing pin on the hammer resting on the primer and not in one of the safety notches. And probably got dropped on the hammer.

I've had the 1 5/8" "convertible" for at least a dozen years. And frequently advise any new onwers of these little revolvers, that yes, at some point you WILL drop it. If handled often, probably more than once. ;-) Those safety notches are there for a very good reason!
