Just kidding. The "belt pouch" works well.
I have a Hoffner's "camera pouch" - it's basically a more square version of a fannypack that you wear on your belt, doing away with the straps of the fannypack.
The use of a "real" gunbelt does a lot for comfort, and with the gun not digging into MWB (my waistbaind), the use of the pouch is the most comfortable of all the choices, however there is a post about a 5.11 "Holster Shirt" that has grabbed my attention.
(Just for the record, I plan on wearing a cover garment over the holster shirt - I'm afraid my gut might print bigtime.)
When I do use the pouch, I feel like I'm almost open carrying.
The Holster Shirt leaves no "hey, what's in the pouch?" questions./////////////////
