I think the big red "51" goes back to some mix-up in wording of the Health Code. Every hospital I've ever seen with a valid 30.06 notice also has the 51 in a circle.
It was explained to me long ago. I don't remember the explanation, and the conflicting statutes have probably been addressed by now.
