If you think about how the extractor and ejector bar work together it makes sense if you go to slow on some pistols that this happens. Ejection needs some momentum to work.

The unfired round might always ride on the edge of the opening but with enough momentum it wont fall off the extractor when that happens. Tipping the gun right helps because the extractor is now under the round and gravity is helping hold it.
