Mine says "Member 1". Great, that should help slow things down! It's the only copy I got I'm sure or they would all have been in the same envelope.

Mine, issued 80/05/23 is smaller than 8.5 x 11 and has sections numbered 1 through 22 (it shows the form version date as 1 July 79). I was never given a long form that I recall (although that was a long time ago). I'll be sending an SF-180 to National Records Center today I guess to see about getting the long form.

I submitted a copy of the DD-214 that I was given in '80 when I was discharged (honorably). I don't see anywhere on it that indicates the type of discharge it was. Am I not seeing it somewhere, and if it's not there, are they going to kick my application back? I'd call them and ask, but you know how that works... :roll:
