The barrel is up at your chest and horizontal it looks like. She would draw by grabbing the grip and pulling the weapon down which would likely cause the muzzle to tilt upward when drawing.

Can't imagine it's the most comfortable method of carry for the average woman, either. Great for the movies, I'm sure, but unless you're CCing in lingerie, it doesn't seem necessary.
