I think I've visited an extraordinary number of medical facilities as of late, both for myself and my folks, and I made a conscious effort to observe the many differences. First off - I saw that most doctor's offices I visited inside a medical building are not posted and stand alone offices are posted. Opposite of what I would have assumed. Specialists such as eye, spine etc all seemed to be posted, although 100% to specs would be questionable on a couple of them. Laboratory services like X-rays and blood work and such are all posted. Some of these waiting rooms were full of shady lookin people, sporting various gang tats. Luckily, 30.06 does not apply to LEOSA or I would have been mighty uncomfortable a couple times. The physical rehab place was prominently posted and I happen to agree that's a best policy there. I could imagine many different scenarios of a pistol falling out of a pocket in there. Of course, every hospital here is posted properly. No exceptions there.
