Okay CHL-6 is on the bottom of the photo id doc. Where can I find the checklist w/ barcode? I don't see it in my binder?

I've already applied online and getting ready to mail the following documents.

1. Certificate of training form.
2. Photo I.D.
3. FAST applicant form

Am I missing anything else? Affidavit?
