Married - how to change name on LTC
A friend recently got married and asked how she changes her name and address on her LTC.
The on-line application site indicates you can use it to change your address, but doesn't say anything about being able to change your name.

I found a link to a CHL-70 that can be used to change name and address, but it's just a form so it would have to be mailed in.

I didn't actually login to the on-line application to see if it also allowed a name change. Does anyone know the answer to that? Or is the mail-in form the only option?
