I am 5'-11" and 175 lb. I carry closer to 3:30. On my hybrid holsters with two clips (Crossbreed, Tucker Answer, C-Tac), I put the forward clip just in front of the 3:00 belt loop usually (although belt loop positions are slightly different on different pants). On my kydex PJholster (single clip), the belt loop is directly behind the belt loop closest to 3:00. This put the slide directly just behind the 3:00 position. That way there isn't much of a side to side bulge showing, and slide is not under my hip or butt while sitting. I carry in this position with an M&P40c (Tucker), a Full Size 1911 (C-Tac), and a PM-9 (crossbreed supertuck and pjholster). The full size 1911 is the closest thing in slide length to your 22. In terms of width, the M&P9c. The M&P9c is the least comfortable to carry for me because it feels the bulkiest. The 1911 is the heaviest, but with the right belt, it is still better (because of the thinness) than the M&P. I have a Glock 22 also and would never think about carrying that at my size. Glock 23 would be about as big as I would try to carry. I have slim grips on the 1911 and the grip on it is the longest, but it's not quite as brick-like as the glock and conceals better IMO.

You don't mention your height, but my recommendation would be to go up in shirt size to a Large. Or, stick with the medium t-shirt and wear a size large (or some mediums if they fit bigger) button down shirt on top of that. There are other threads here with pictures to characters on TV (My name is Earl, Big Bang Theory, etc) wearing the layered two shirt look. I used to wear a medium t-shirt before carrying. I use large now and sometimes i don't feel like they are big enough either. With the layered look, I rarely worry unless the shirt is shorter and i am having to do some kind of gymnastics at the store to retrieve something high or down low or whatever.

I do not wear baggy jeans. I usually wear slim fit boot cut, but I have experimented with some more relaxed fit jeans with varying degrees of success. The higher you can put the waistband the better also. Though I prefer lower rise pants traditionally, I have found the slightly higher waist on the relaxed jeans does a better job of holding the pistol closer to the body.
