OK, this may be an indication of me getting old and senile, but here's what happened to me:
I registered and paid for the CHL online. When I finally got the application materials about a month later, I could not figure out how the photo holder was supposed to work. Then I went to my CHL class the next day, and I found that the photo holder sent in my packet did not have any perforations on it; no place to separate the form and no place to punch out the opening for the photo. I thought that was strange; the instructor said he had seem a few like mine recently and gave me a good one. I didn't think much about the possibility of any other glitches in my materials after that.
Before I mailed my completed application to DPS, I was very careful, anal even, about getting all my forms completed and correct before I mailed them, double- and triple-checking both sides of everything for complete info and signatures. I made a copy of everything before I mailed the packet, which involved flipping over each page of the 2-sided application form itself to place each side on the copier.
About 30 days after DPS received my packet, the status went from 'Packet mailed to applicant' to 'Application incomplete - missing information - detailed letter has been sent'. I couldn't believe it. I sent them an email asking what the problem was, to which they replied "You didn't sign the web form". I thought "What the heck is the web form?" and went home to look at my copies of everything, but still didn't see any missing signatures on anything.
A few days later when I got the letter from DPS requesting the missing info, I realized that there is a fourth page to the 2-sided application, with a signature line, which I did NOT have in my copies.
Like I said, maybe I overlooked it due to a senior moment, but I am convinced that the application they sent me was BLANK on the other side of Page 3. Now I have to start the wait all over again. Grrrrrrrr!
Make sure you have 4 pages to the online application form, which is different from the form an instructor will give you in class, because the last page requires your signature!

Slight clarification
On the two photos - one must be signed by you and the fingerprinter, and it's a good idea to paper clip it together with the 2 fingerprint cards. The other photo should not be signed and should be mounted in the holder.
Also, not sure about this, but my instructor told us the felt tip pen for signing the back of the first photo was not really required, but was a holdout from years ago, when perhaps some pens couldn't make a permanent signature on the back of official passport photo paper. Kinda makes sense, i.e. if you get a good permanent signature in black ink, how could anyone tell whether the pen was a felt tip or not? However, please don't take my word on this!
