I was wondering for people who carry semi autos if they keep one in the "pipe", I currently carry a revolver in the summertime but in the winter when I am able to wear a coat I carry my semi. I do keep one in the pipe I was just wondering how many of you guys do.

Yea buddy. 1911 Cocked & locked the way they were designed to be. All others nedd to have one ready to go. If you need your gun, your other hand may be busy fighting.

Always. My 1911 is a rock without one in the chamber.
Condition 1 is the only way.

I carry with a round in the chamber - - - it's too easy to forget to rack the slide.

Yes. ALWAYS.
You will not have time to rack a slide when (if) the time comes to protect yourself. You can bet the BG has a revolver or has one in HIS pipe when he approaches you.
Kinda like going fishing without any line on your reel, can't do much with either tool if it is not ready.

If the gun is with me or at home for defence, then there is one in the hole ready to go. Only time that there is in not is if its in the safe, but this does not happen very often.

Absolutely.
I don't believe I'll have the time OR ability to draw, rack the slide, and shoot while trying to push an aggressor away with my other hand.

Always.
And all guns in my home remain loaded, unless they are being cleaned.

Fully loaded or fully empty. If it doesn't have one loaded in the chamber it doesn't have a loaded magazine inserted either.
Mostly it is TV for the cops and others to chase bad guys all day but jack the slide when they get shot at (for dramatic effect.) Or another favorite: the sound of thumb cocking a loaded Glock.
