Name Change

Hi,

Since the time my wife filled out her CHL apl on line and did her fingerprint, her last name has been changed due to marriage, That's to me! . She has not taking the class yet and wondering how we should go about the new last name of hers. Can she include documents along with the packet to Austin?

Yes, she needs to include a CHL-6 & CHL-70 forms. Also send a copy of the marriage certificate/license. Do not send these in seperate, take the class first, then mail all your items on together at the same time. You cannot edit nor change your online info either. DPS will do that on their end once they get all your documents.
