address/name change on CHL

I recently got divorced and I need to change my current address as well as my sir name from my married name back to my former maiden name as stated in the divorce decree.I've looked online at the DPS site and only found an online address change form,but it states on the site than name changes have to be done by the conventional paper method via mail but it doesnt give a link to the printable forms nor does my local DPS office have these forms.I emailed DPS and have rec'd no response.Can anyone point me in the right direction? Maybe I missed it somewhere.Thanks in advance! :oops:

Here it is. Also, with a name change, you'll need to send a photo-holder signed with your new/old name.

That's the long, narrow form that you signed and then taped your photo under the flap. You apparently don't have to send new photos, but they do need your signature using your new name.

Ah ok! Where can I get one? there isnt a range around here for miles that I know of.Maybe the local DPS office?

If nothing else, you can probably get the holder at a local wal-mart or sears portrait studio... any place the does passport photos. Also, you might check your local post office, as some of them do passport processing.

Most Walgreens will provide this service and supply a holder with photos as well.

Not to mislead... only photo services are available. Also, Kinko's/FedEx can take acceptable photos.


My wife is having a similar problem with changing certain forms for name change. The 'in person' problem comes up frequently.

Well,after asking here the DPS emailed me back and they sent me all the correct forms and a photo holder free of charge via USPS.Sweet! :cool:

Nope! I left him.I pulled up stakes while he was at work and nothing of mine was left when he got home that night.I DID however leave EVERYTHING that was his including all HIS guns and reloading stuff etc etc etc.The only thing I left there that was mine was my dog and she was closer to him anyway so I got a new one when I moved out.Now hes telling everyone how he got shafted.Whatever! I'm on my own,back in school,getting a job and MUCH better off w/o him.

Don't worry about what he tells others. It sounds like you're doing what is best for you, and that's all that should matter.
