I am not sure that she listed the arrest when completing the application online. If so and if she explained the situation, it may have been fine. The other form still would have likely been needed, but maybe it would have helped them find what they needed to find. I would certainly think they would have been able to figure out it was in a different state and find the details on their own. Send a letter with questions like they did or call. I thought it was funny that they showed it being in Texas when it clearly was not. They got the city name right, but wrong on the county and state. So I got the letter a couple days ago about supporting documents. I uploaded everything, hopefully it doesn't take 60-120 days. The lady said eligibility has to check it then they will ship it out. Hopefully no more hiccups.

Speaking for my end, we have to request:
bill of information(charges)
disposition of minutes(null prossed)
include a self addressed stamped envelope and $13.50
The above once received from the other state along with the Texas form CHL-78C.
That should cover the additional supporting documents needed.

I just uploaded my court minute and another paper. Crazy cause the papers say too different charges. Even the county I went to jail in doesn't have the charge DPS pulled up.

Yea I have no idea what happened with it. Just sent in the abstract from Mississippi for my case from back in 1993. I didn't wait for the letter, but I'm sure one is on the way. Let's see if that's what they were waiting on.

My status changed today from status pending to mailed I had an unusual wait time. Everything received by DPS 02/15/16, last week of May I got a letter wanting court records of something stupid I did in 1987, mailed that 05/31/16 certified mail. Waited, and waited, and waited! Called DPS 01/17/17 for them to tell me that they never received my court papers, I'm thinking the mail man delivered it to the wrong building or something, I don't know, but lesson learned, I will never mail anything to DPS again. Electronically sent them the second time. Status changed from Background under review to print pending on the 20th, to mailed today. So happy this is all over with. I was starting to wonder if they had to go looking at my dental records or something there for a few months. Very nice lady at DPS, she said technically they should have cancelled the application after 90 days from not receiving the court records and then updated the module to make sure they wouldn't cancel it after I called in.

Right at 60 days finally all clear and at print stage. The website is not accurate. They told me on Tuesday background was complete (last line I was waiting on). The website did not update till yesterday in the middle of the night. Waiting on plastic!!!

5 month update. They finally accepted my fingerprints.
