Pre-printing CHL-100's

I would like to have some three part carbonless CHL-100's printed up. Local print shop can do them cheaply enough to suit me.
Is it OK if I have them pre-printed with my name and instructor number on them, but leaving a blank for me to sign after class?
I had a bunch of the old forms that I bought from someone, and they worked great. Had to throw a bunch out when the new forms came in.
Thanks in advance.

Yes. That is what I use, but I print them myself. The work great in the $99 HP laser printer I picked up. I don't print too many at a time due to varying classroom / range locations though. Don't forget to put the hours per subject and total hours. I picked up the 3 part NCR from Fed Ex / Kinkos. I agree, if you do not have access to a printer at the classroom / range location they work great!

I have everything pre-filled out (including my signature) except the student name, DL #, and two dates.

The night before class, I just fill out the names, DL, and classroom date. Then when the classroo0m portion is about over, and I can see that the weather isn't going to keep us from the range, I fill out the range date.

I use an Excel spreadsheet. Prints the CHL 100 and the CHL8.

I use the excel spreadsheet as well and just print out 3 copies for each person in the class. I just don't sign either section until the student passes.

On the Excel Spreadsheet, has anyone been able to add an extra line for an additional instructor ? I'm need 2 instructors, and maybe reduce the student lines to 25.

It is my understanding that there can be only one instructor for each section (class or range), so not sure why would you need an additional instructor line. Even if there is another instructor "assisting" with logistics, there can be only one official instructor, and s/he would be the one to sign the form.

I was under the impression every instructor was required on chl-8 ? On 100, I sign on classroom portion, while my co-instructor signs off on the proficiency.

I'm sorry... I didn't see that the subject had changed from -100's to -8's.

Technically, I think, since one of you is the instructor for the classroom portion and the other for the proficiency portion, each of you would submit a CHL-8 in your own name (much like you'd do if a student came to you independently for just one portion).

If DPS has been accepting -8's with double signatures from you, I'd be happy to modify the spreadsheet to handle it. Or, it could be made to produce two of them, one for each of you.
