Anything 20" and up is going to be pretty good with practice. In 3 gun I shoot steel plates at 475 using an 20" barrel without much trouble.

.38/.357 is one you have to be really careful reloading. It's easy to double or even tripple charge without overflowing the case.

I have taken my young one (2 yrs old) to her doc many times and we have never been asked this question. I guess it just varies from doc to doc.
