I actually did this one time myself. I had a Springfield 1911 chambered in .45 and a Para-Ordnance P-16 chambered in .40 S&W. I was firing them both and loaded one round of .40 in my .45. Same result. I had to pry the expanded cartridge out. Now adays I'm mindful of what I'm doing. I also have a Kimber Pro-Carry chambered in .45 and one chambered in .38 Super that look almost identical. One weekend I was out shooting and I loaded up a .38 Super mag, inserted it to the pistol and went to aim when I saw the round hit the ground. Yup I put the .38 Super mag into the .45 released the slide and launched a whole round down the barrel instead of just a bullet! Want to know the really sad part? I did it again (racked the slide that is) and watched another one fall before I said to myself "DUH! " and took the magazine out. I have since put very different colored grips on the pistols to tell them apart.
